How to Book a Consultation:

- Book an online consultation by choosing a time slot. *All bookings should be made 24 hours in advance. Appointments made in less than 24 hours cannot be accommodated.
- Go through the checkout process to confirm your appointment.
- Each consultation lasts for 30 minutes and will start promptly.
- Sign and send us a copy of the TELEDERMATOLOGY CONSENT FORM before your appointment. We will not proceed with the consultation without the consent form.
- For In Person Consultations, please fill out the Health Declaration Form.

Terms and Conditions
- All bookings should be made 24 hours in advance. Appointments made in less than 24 hours will not be accommodated.
- If you do not show up for the appointment, your booking is forfeited.
- You may book again for a later date/time.
- One person per booking only.
- Consultation will be done via Zoom video call.
- Recording of the consultation is NOT ALLOWED, and we will not keep a video record of the consultation.
- Online consultations do not replace the actual visit to the doctor and complete physical examination of the patient
